  • Fill out the form with the required information and do not forget to check the “record review” box at the top of the page.
  • Pick a live scan location for fingerprinting from the list available here.
  • Take a valid ID card along when visiting the live scan center. Fingerprinting fees vary among service providers, and you can call and inquire about the latest fees.
  • Get a money order or make out a check of $25 payable to the Department of Justice. This is the fee for record processing.
  • Once you have your fingerprint card with your name, date of birth, sex, and return mailing address on it, mail it along with the check or money order to the California Department of Justice, Record Review Unit, P.O. Box 903417, Sacramento, CA 94203-4170.
